Your local EMS Provider provided services that are generally billable to your insurance. Please provide insurance via the contact form or call us for more information.
Your taxes may subsidize some services provided by the local EMS Provider, but do not cover all the costs associated with the service.
Most cases will qualify for an approved payment plan which can be set up by calling the Customer Service team, while the account remains in house. Some cases will be reviewed and considered for hardship based on town billing guidlelines, balance of the bill and supporting documentation presented by the patient. Please contact our Customer Service to determine how we could further assist.
LSF: Lifetime Signature Forms These forms are sent to the patient with active Medicare insurance and/or of Medicare age. The signature is imperative to obtain per Medicare regulations as well as to bill to the payor. The patient is required to sign and date the form. In the event the patient is unable to sign, there is a field for an alternate signature. This must be signed by a relative or legal guardian, dated, and with an acceptable reason as to why the patient could not have mentally or physically signed the form.
To find out if your insurance has been billed, please call Customer Service with your account number to verify that we have the correct insurance on file. For motor vehicle incidents, please have your auto insurance ready.
Please allow up to 7 business days to process the received insurance information. Once received and verified processing with insurance takes an average of 30 days. You may always contact your insurance to confirm the bill was received within the 30-day processing time.
Denials are reflected on your billing statement when an insurance company refuses to pay, applies the balance towards a deductible or coinsurance or they need further information to process your claim. Please check your explanation of benefits and call 630-530-2988 for payment options.
